It is a common form – and a common problem. Sometimes someone will need a power of attorney and will sign that – maybe not under duress, but because they have no other options at that moment.
Or maybe a relationship will change at some point and the party does not want to grant another party the power of attorney anymore. So if you decide to change your power of attorney at some point it’s not just a case of appointing a new power of attorney. You need to also revoke the privelages of the previous power of attorney. That way no situations arise where two people are attempting to represent the person with their own power of attorney form.
Filling in our power of attorney revocation form is extremely simple to do.
The first thing you do is fill in the Principals detail. The principal is the person who will be granting the power of attorney, who previously granted the power of attorney, and is now revoking said power of attorney.

Next you can list the agents – which is the new power of attorney who is being assigned as the power of attorney for the Principal.
Next, you will list the date of the power of attorney contract. This is the original power of attorney contract which you signed – the one of which you are revoking. By listing the date it also helps identify the specific power of attorney agreement.
You may also wish to attach the original power of attorney agreement if you have a copy of that. We have given you that option. Although it is defaulted to no – we do highly recommend attaching a copy just to make everything as clear and concise as possible.
You can then choose if the power of attorney is durable or non-durable.
